  7. A document confirming the right to change the surname, if available (for example, the child's father changed his surname and now wants to change the surname of his child).

If the child is between the ages of 7 and 14: 

  1. 2 photos 3.5 x 4.5 - of a child;
  2. A child's birth certificate;
  3. Passports of the child's father and mother;
  4. Individual tax number of the child's father and mother;
  5. Marriage certificate of the child's parents;
  6. Personal presence of both parents and a statement of consent to change the child's surname / if one of the parents cannot be present at the Civil Registry Office, a notarized consent from him/her is required / a document confirming that one of the parents has been deprived of parental rights;
  7. The child's personal presence and a signed statement of consent to change the surname.

If the child is between the ages of 14 and 16 (the child may change his or her surname at will, but the consent of one of the parents is required)

  3. Passport of the child's citizen of Ukraine;
  4. Extract from the Unified State Demographic Register on the registration of the child's place of residence;

REGISTRATION PROCEDURE

RNOKPP (Registration number of the taxpayer's account card) is a unique identification number that is issued to every citizen of Ukraine for registration in the tax system. It is also known as an identification code or tax number. This document is required for official employment, opening bank accounts, registering business activities and other financial transactions.

    Stages of the procedure for changing the surname (first name or patronymic)

    To begin with, we will need to prepare all the documents:

    1. The following documents are submitted to the civil registry office: 

    The applicant's birth certificate; marriage certificate, if any, and a certificate of divorce or a court decision on divorce; birth certificates of minor children (under the age of 18); if there was a marriage, you must also submit extracts of the premarital surname for each marriage; certificate of name change (if such a procedure has already been done).

    1. Identity documents - it should be noted here that only citizens of Ukraine can undergo the name change procedure in Ukraine. In this case, it will be the Passport of a Citizen of Ukraine or the child's birth certificate and the passport of one of the parents (the applicant) + the notarized consent of the other parent. If the documents for changing the child's name are submitted by a single parent, a document confirming this must be attached.
    2. Certificate of residence registration. Although it does not matter where the applicant is registered during martial law, as the documents can be submitted to any registry office, the registration itself is mandatory.
    3. The applicant's photos of 3.5 cm x 4.5 cm - 6 pieces - are attached to the case file.
    4. The applicant must be present at the time of submission and receipt of the certificate.

    The next step is to transfer the name change case from the registry office to the police for verification.

    At this stage, it should be noted that the applicant's criminal record will be checked first. This check takes most of the time to complete the name change procedure.

    We would like to add that if the applicant's birth or change of surname by marriage was not in Ukraine, the Civil Registry Office will send requests under international treaties to the relevant authorities in such countries, which may also take time. 

    The final stage

    Change of the child's last name or first name, patronymic

    Parents, by mutual agreement, may apply for a change of surname or name of a child under the age of 7. After reaching the age of 7, the child's presence is required, as he or she must give his or her consent to the name change. If you need to change the name or surname of a child who has already reached the age of 14, you must first obtain a Ukrainian passport, and only then carry out the name change procedure, and the presence of the child and both parents or one of the parents and a notarized consent from the other will be required. 

    If the child's name or surname is changed in the first years after birth, and such changes are made on the basis of a change in the surname or name of one of the parents, a second birth certificate will be issued, which will already contain the new data of the parents and the child, in all other cases, the registry office issues a certificate of change.

    It should also be noted that the patronymic of a child under the age of 18 can be changed only on the basis of a court decision on deprivation of paternity or if, when issuing the child's birth certificate, the mother indicated the father's information on the basis of Article 135 of the Family Code of Ukraine, namely, according to the mother's words.

    This procedure is carried out exclusively at the personal request (except in cases of changing the surname of a child under the age of 7), who has expressed this desire. The period for consideration of the application is 3 months from the date of its submission and, if there are valid reasons, may be extended for the same period, but not more.
